Our referral
process
We welcome referrals from local authorities, schools, healthcare providers and other professional support services.
We encourage you to contact us to discuss your needs before submitting a referral.
We do not accept referrals directly from parents unless you are in receipt of an EOTAS package. However, we’d be happy to discuss your circumstances and see if we can support you and your young person.
Our process
We support children and young people aged 4 to 25 with special educational needs and disabilities (SEND) and social, emotional and mental health challenges (SEMH).
Referral
Contact us to discuss your needs, and we’ll provide a referral form for you to complete.
Assessment
We’ll assess the young person’s needs and develop a suitable package of support.
Bespoke Intervention
Our dedicated practitioners will develop a detailed intervention plan to address the needs and interests of the young person.
Delivery
We will then deliver the package of support, regularly assessing needs and progress.
Destination
With our support, the young person will take the next steps in their education, whether that’s returning to school, college or a new pathway.
